First Time Appointments
If you would like to book an appointment to see Dr. Scarlett at Coral Springs Dental Centre, please be prepared to complete the Medical/Dental History Form. You may also request the form to be emailed to you ahead of your appointment and have it completed upon your arrival. This will speed you through the system.
